HOW TO BOOK

We book via region (listed below) by email or telephone on a first come first serve basis:

Region: Maple Ridge, Pitt Meadows, Port Coquitlam, Coquitlam, Port Moody, New Westminster

Fee Schedule (Effective January 2014)  

Physiotherapy

  • Initial Physiotherapy Assessment (one body part/joint) - $120 (including telephone consultation, treatment and driving to home/site.  NOTE: If travel is > 25 km a nominal fee may be applicable) 
  • Physiotherapy Subsequent Visit - $ 100
  • Physiotherapy Athlete Movement Screen- $150
  • Home Exercise Plan: PDF Document with visual aides and description.  PDF is constructed within 3 business days subsequent PT assessment and/or treatment: $40/month OR $15 per exercise plan per visit (lesser of the two). 
  • Ergonomic Assessment or Jobs Site Visit (JSV) - $100/hr (typically 2-4 hrs)
  • Aqua Therapy Visit - $120 (30-45 min)
  • Medical Legal Report - $120
  • Progress Report/Physician's Progress Report- $50
  • Fees : $1 dollar per page for photocopy and administrative fees. 

 

Kinesiology

  • Kinesiologist Individual Training Session - $80
  • Kinesiologist Group Training Session -

                              i) $125 (up to 4 people)  

                              ii) $25 per additional person (up to 10 people)

  • We are a flexible service that will work around your schedule not ours! 
  • Initial telephone consultations are free! 
  • Upon request we provide a reminder via email or phone call/text prior to your first appointment

 

CANCELLATION POLICY

Out of respect for each other’s time, no-shows or cancellations within 24 hours will be charged 50% of your scheduled appointment fee.  If you are unable to attend your appointment, an email, text, or phone call is required.   We do understand that emergency situations can occur, and will deal with each on an individual basis.
